Registers under the Telangana Shops & Establishments Act, 1988
The Shops & Establishments Act is the register set most Telangana employers meet first. It covers shops, commercial establishments, offices, hotels and restaurants — which in practice means most businesses in Hyderabad that are not a registered factory.
The Rules made under the Act prescribe forms numbered in Roman numerals, I to XXVII. Five of the records below carry their number here: the Register of Employment is Form XXII under Rule 29(1), the Register of Leave is Form XXV under Rule 29(6), the Letter of Appointment is Form XXVI under Rule 30, the Register of Appeals is Form XIII under Rule 21(2)(a), and the periodical return is Form XXVII under Rule 33.
The others are named for what they record rather than by a form number, and the reason is worth being exact about. For some of them a prescribed form does exist — the Rules put the register of wages at Form XXIII and the register of fines at Form X — but naming a sheet by its form number is a claim about its columns as well as its purpose, and until the columns conform, that is a claim CORGIS will not make. For the rest, no numbered form under these Rules answers to the record at all. Either way a register named for what it records is telling you the truth about itself, which a number that appears nowhere in these Rules was not.
Most of the set is monthly. The leave registers, the Leave Card, the appointment letters and the year-end summary are compiled for the year, and the periodical return is due four times a year. Form XXV is not one register but five, generated separately for casual leave, sick leave, earned leave, loss of pay and compensatory holiday, so each reads as its own register rather than five sets of columns competing on one sheet.
Every figure below is computed from the attendance and payroll data already held for that establishment. Nothing is re-keyed from one register into another, which is where most register errors come from — one wrong figure in a wage register reappears, uncorrected, in the year-end summary that draws on it.
Who keeps this register set
Shops, commercial establishments, offices, hotels, restaurants and theatres in Telangana. Establishments registered as factories keep the Factories Act set instead, and an establishment engaging contract labour keeps the Contract Labour registers alongside these.

The roster of who is employed at the establishment — the register every other one is reconciled against. Built from the employee records held for that company, so a new joiner or a leaver appears here in the month they actually joined or left.

What each employee earned for the month and what was deducted, laid out with the wage components configured for that company. Earnings come from the computed payroll for the period rather than a typed figure, so the register and the payslip cannot disagree.
FinesRegister of Fines
Fines imposed on employees, drawn from the wage-deduction ledger. Each entry carries the act or omission recorded against it and the amount, and stays linked to the recovery it produced in the wage register.
OvertimeRegister of Overtime
Overtime worked in the month, per employee, with the hours and the payable amount. Hours come from the attendance grid, including the from/to times recorded against each overtime day.
Wage SlipsWage Slips
One wage slip per employee for the month, showing days worked, payable days, earnings and deductions. Generated as a single workbook with an index sheet that links to each employee.
Leave CardLeave Card
A per-employee card of leave taken and earned across the year. Earned leave is accrued from days actually worked rather than assumed, and capped at the allocation configured for that company.

Year-End SummaryAnnual Return
The establishment's own consolidated summary of the year, compiled from the twelve months of attendance and payroll already recorded rather than assembled by hand at year end. The Rules do not prescribe it and no form number belongs to it — it is a management record kept for the employer's own file. The return the Rules require is the periodical return below, and this sheet does not stand in for it.
Form XXVIIPeriodical Return
The return the Rules require, and the one an establishment is most likely to under-file. Rule 33 asks an employer with ten or more employees to send a return for each of the months ending March, June, September and December, so as to reach the Inspector not later than the tenth day of the month following — four returns a year, not one, and the December return falls due in January of the year after. The sheet states the quarter it covers, the rule it is filed under and the date it is due, and marks itself interim if it is generated part-way through a quarter. One limitation is worth stating plainly: the gazetted Form XXVII proforma could not be obtained, so the figures are laid out in CORGIS's own format for transcription onto the prescribed one, and the sheet says so.

Form XXVRegister of Leave
The leave register, prescribed as Form XXV under Rule 29(6). CORGIS produces five of them rather than one, generated separately for each leave type — casual, sick, earned, loss of pay and compensatory holiday — so each reads cleanly on its own. Leave is grouped into actual instances: a Friday-to-Monday absence bridging a weekend is recorded as one four-day instance, not two separate entries.
Form XXVILetter of Appointment
Appointment letters per employee, prescribed as Form XXVI under Rule 30 and drawn from the employment particulars and the salary in effect at joining.
Form XIIIRegister of Appeals
Appeals recorded against the establishment, in the register prescribed as Form XIII under Rule 21(2)(a). Entered on the Compliance Data screen and rendered into the register format.
Inspection BookInspection Book
The inspection record for the establishment — visits, the officer, matters examined and directions issued — kept as a running book rather than loose notes.
All fifteen are produced as Excel workbooks for the period you choose, and can be downloaded individually or as a single ZIP organised by Act — nineteen files in all, since the Register of Leave is generated separately for each of the five leave types. Registers that draw on manually-entered particulars — the display notices, the appeals register, the appointment letters and the inspection book — render from what has been entered on the Compliance Data screen, and print the blank format until it has been.
